Nutrition Counseling and Meal Planning

You and your family* can receive nutrition counseling and meal planning services through the ake CARE Employee Clinic's on-site Registered Dietitian and Diabetes Educator.

You can receive help with:

  • Managing your weight and chronic conditions like hypertension and diabetes
  • Eating more healthily

Your Take CARE Employee Clinic provider may refer you for nutrition counseling, or you may self-refer.

You’ll pay just $10 for these services if you are covered by a CM medical plan. Otherwise, you’ll pay a market-competitive rate.

*Your spouse and adult dependents ages 18 through 26 also are eligible for nutrition counseling/meal planning at the Take CARE Employee Clinic and Pharmacy.
