Appointments, Hours and Parking

Note: The Take CARE Employee Clinic and Pharmacy accepts only credit, debit and Flexible Spending Account (FSA) cards. The center does not accept cash.

Before you call

If your spouse or adult child (age 18 through 25) is not covered by your CM medical plan, you must add his or her name to your Infor dependent record before he or she contacts the center to schedule an appointment:

  1. Using a CM computer, log into Infor Talent Management using your network username and password

  2. Double-click Employee

  3. Click Employee Profile

  4. Click Personal Information

  5. Scroll down to add dependents

Cancellations, late arrivals and no-shows policy


  • If you need to cancel your appointment, call (816) 412-2355 two or more hours before the scheduled appointment time.

  • If you are 10 or more minutes late for your appointment, you may be asked to reschedule.

  • If you do not show for your appointment, you will be considered a no-show. Patients with three or more no-shows may be dismissed from the center.

Transferring Medical Records

To have your medical records transferred to the Employee Wellness Center, submit a Medical Record Release Form to your previous health care provider(s). Providers may ask you to complete their own form as well.


The center is open five days a week, including:

  • Mondays through Thursdays from 7 a.m. to 6 p.m.

  • Fridays from 7 a.m. to 5 p.m.

To reach a provider after hours for immediate urgent care needs, please contact our answering service at (913) 338-8247. Refill requests will not be granted by the on-call physician. For emergencies, please call 911 or visit the nearest Emergency Room.

The center is closed on Saturdays and Sundays, and will be closed on the following holidays: New Year’s Day, Martin Luther King Jr. Day, Memorial Day, Fourth of July, Thanksgiving, day after Thanksgiving, Christmas Eve, Christmas Day and Labor Day.



  • The center is located at 2401 Grand, Suite 100, Kansas City, Missouri 64108.

  • Ride the Crown Center shuttle from the Main Campus OPC circle drive to the first stop at 2400 Pershing, and walk to the center at 2401 Grand, near Panera. The shuttle is available from 6:45 a.m. to 6 p.m. Monday through Friday.

  • If you drive, enter the parking garage at the intersection of McGee and Pershing. Stay in the right lane to enter the reserved parking area. Bring your ticket in to be validated. Enter through the 2401 Grand glass doors; the center is directly in front of you.
