Patient Portal

The Children's Mercy patient portal provides you access to your child's health information 24 hours a day, 7 days a week. You must complete an electronic authorization form to request access to the patient portal.

Benefits include:

  • View lab results

  • Message your care providers

Not enrolled? Disconnected?

Steps to enroll or re-enroll in the patient portal:

A. Fill out the Authorization for Participation in MyChildrensMercy Patient Portal. You can get a copy of the form by:

B. If you are a patient over the age of 18: You will need to include a copy of a signed form of identification (like a driver's license or school ID) with the application. If you wish to give your parents access, sign the proxy section.

C. If you are a parent of a patient over the age of 18: You will need to include a copy of legal documentation with the application stating that you are the court appointed guardian of the patient. The patient may also grant permission to you to access their account.

D. Return the forms. You may:

  • Email them to

  • Mail them to Children's Mercy Health Information Management - 2401 Gillham Road, Kansas City, MO 64108

E. You will receive an email invitation to the patient portal when the forms are processed. You must accept the invitation. You will be asked to change your password when you first log on to the portal.
