Implement the patient centered medical home model to ensure comprehensive and coordinated care to pediatric patients

  • Beacon Program is seeing 100+ complex, chronic, special healthcare needs patients and their siblings.

  • Progress in Primary Care moving towards medical home 

    • 12 sites (9 practices) that are now NCQA recognized within the PCN (7 added this past year) 

    • Children's Mercy West is NCQA certified and reapplying this year

    • Interactive enhancements made to the CMPCN Portal to improve provider usability

      • PCP panels, disease registries and ER utilization, and more

    • Starting a learning collaborative to include 1-2 webinars per month on provider focused topics 

    • Social Worker has been added to each PCC team

  • Medical neighborhood is a work in progress

  • Implemented use of SEEK Tool in PCC Clinics to identify families with psychosocial barriers to engagement in care
