New Hire Documents

Complete all new hire documents one week before your start date

 

In your offer letter, you were asked to complete all new hire documents at least one week before you attend Children’s Mercy Orientation (CMO), which is on your first day of employment.  

New hire documents collect information that is needed for:

  • Your employee record

  • Your parking permit

  • Your background screening visit 

    • The Health Form and the Fit Test Health Questionnaire, if applicable, must be printed and brought to your appointment at the Employee Wellness Center.

  • If applicable, to grant access to Medical Records. 

    • The Medical Records Signature form must be printed and brought to CMO.

You received a separate email from service@validityscreening.com asking you to complete section one of your I-9, which is a federally required form used to verify your identity and legal authorization to work in the United States.
