Collaborate with community agencies to establish a high-quality, measured home visitation system.

  • Home visitation program “Project 1,000…” (Note: visits by pilot project agencies to start in 11/1/14)

    • Obtained $175,000 external funding for the program

    • Database for centralized referrals intake and scheduling built

    • Centralized Database built and ready for use (captures data f0rom each home visit) – RFP for 2 to 4 agencies to use in a pilot project (submissions due 8/29/14, notifications 9/12/14, training commences 10/1/14, go live 11/1/14)

    • Full meeting for entire network of 12 participating metropolitan home visiting agencies and other associated entities - 2x per year for ongoing collaboration; intermittent updates and communications about Project 1000 by email

  • Note: CMH Home Visiting, Dept of Social Work is now a Healthy Families of America Affiliate. Application submitted by Dr. Moffatt, Oneta, Heather, etc. has been approved by HFA. The CMH Home Visiting Team, who formerly used the Healthy Start model, will transition to the Healthy Families America model of home visiting.

  • Established formal Memorandum of Understanding with the United Way of Greater Kansas City to jointly develop a collaborative home visiting network
