Goals

  • Identify patients at risk for severe sepsis

  • Completion of first antibiotic within first hour

  • Completion of 60 ml/kg within the first hour

  • Vascular access within 15 minutes

  • Blood culture obtained prior to antibiotics

Metrics

  • Time to IV access

  • Time to aggressive fluid management completed (40 mL/kg, 60 mL/kg)

  • Percent of patients who received aggressive fluid management in the first hour

  • Time to first antibiotics

  • Percent of patients with blood cultures sent prior to antibiotics

  • ED length of stay

  • PICU length of stay

  • Hospital length of stay

  • Multi-system organ failure 0, 24, 48 hours

  • Mortality

  • Missed ED patients defined as any patient with severe sepsis or septic shock in PICU within 24 hours of their ED stay

These guidelines do not establish a standard of care to be followed in every case. It is recognized that each case is different and those individuals involved in providing health care are expected to use their judgment in determining what is in the best interests of the patient based on the circumstances existing at the time. It is impossible to anticipate all possible situations that may exist and to prepare guidelines for each. Accordingly these guidelines should guide care with the understanding that departures from them may be required at times.
