Other Documentation Requirements

In addition to filling out the vaccination form, you will need to present us with the following documentation:

  • Documentation of two negative skin tests or one IGRA – either Quantiferon Gold or T-Spot lab test – within the past 12 months
  • Proof of immunization or immunity to the diseases listed on the form (chicken pox and MMR) in the form of a titer. For instance, if you say you had chicken pox, then you will need to have a titer drawn (blood test) that shows you are immune to the disease. If it comes back negative, you will need to have the series of two varivax shots. We will need that documentation. The same goes for your MMR (measles, mumps and rubella) vaccination.
  • Documentation of a seasonal flu shot (if observing during flu season)
