Survive & Thrive Care Team
The care team within the Survive & Thrive Clinic includes:
Physicians/advance practice providers: Your child’s provider will complete physical exams, including labs, x-rays or other tests that may be necessary. They can also consult with specialists and provide referrals as needed.
Dedicated Survive & Thrive nurse: This experienced nurse will be your family’s primary, day-to-day contact throughout the duration of your care. The nurse is always available to answer any questions you may have about any aspect of your child’s treatment.
Transition nurse navigator: The Survive & Thrive team, in collaboration with the University of Kansas Health System and the Masonic Cancer Alliance, has developed the Survivorship Transition Clinic. This clinic is designed to meet the ongoing health care needs of young adult survivors who have transitioned. The nurse navigator works jointly with the Survive & Thrive Program and the Survivorship Transition Clinic.
The transition nurse navigator and the Survive & Thrive nurse will help your child prepare for the transition to adult care when they are 15 to 16 years old, with a goal of developing a plan for transition of care once they are 21 years old. The treatment summary and recommendations for follow-up are shared with the primary care provider. The nurse navigator will serve as a bridge for those survivors that choose to transition their care to the Survivorship Transition Clinic, and will continue to meet their educational needs and serve as their day-to-day contact. Survivors have a choice of where to continue their follow-up care and the Survive & Thrive team will work to develop a plan that best meets each survivor’s needs.
Social workers: A social worker can be available to help you find resources, and can also provide emotional support to help relieve any emotional distress you feel. The social worker will also make appropriate referrals to outside community resources if necessary. They can also facilitate referrals to psychology for neurocognitive evaluation.
Nutrition assistant: A nutrition assistant will help evaluate your child’s nutritional needs. The nutrition assistant will work closely with you to develop the best diet for your child and provide nutrition advice and resources when necessary.
