Allergic rhinitis, anaphylaxis, angioedema, asthma, atopic dermatitis, food and drug allergies, immunodeficiency, immunotherapy, latex allergy, sinusitis, stinging insect allergy, urticaria

For anaphylaxis, please indicate on referral form if patient has an EpiPen.
