Please complete and attach the Sleep Disorders Center New Patient Referral Worksheet and submit with New Patient Appointment Form.

The Children’s Mercy Contact Center will call the patient within 2 business days to schedule an appointment.
Scheduled and requested appointment status can be found on the Wednesday referral reports.
Follow up appointments should be scheduled by the patient by calling the Sleep Medicine Center at (816) 983-6490.
Craniofacial malformations, Down Syndrome with sleep disturbance, frequent awakenings, narcolepsy, observed obstructive sleep apnea, restless legs, severe snoring, anxiety/worry surrounding sleep, bedtime refusal, inability to sleep without parental presence, insomnia, nighttime fears or nightmares, sleep delays, sleep hygiene problems
This clinic requires that a physician's referral be submitted prior to scheduling an appointment.
Please be advised that the patient's initial appointment will be a consultation only.
