Scheduled and requested appointment status can be found on the Wednesday referral reports.

This clinic requires that a physician's referral be submitted prior to scheduling an appointment.

For concussion, please indicate on referral form the date of the injury as well as any current symptoms.

For syncope, patient should be seen in the Cardiology clinic prior to being scheduled in Neurology.
