The Healthy Lifestyles Initiative (HLI) is designed to increase individuals practicing healthy lifestyles and at a healthy weight in the Kansas City area. This is done in collaboration with partners using proven methods, building on current resources, integrating across sectors (Community, Healthcare, Government) and tracking what works. 

What We're Working On

  • Increase community engagement and collaboration.
  • Expand supports for new partners to incorporate HLI elements within their programs. 
  • Increase the number of partners engaged in HLI.
  • Increase the number of children and families who hear the message (12345 Fit-tastic!) and receive education on targeted behaviors. 
  • Increase the number of children and their families who receive a healthy lifestyle assessment and customized plan through primary care settings or health screenings. 
  • Improve linage between partners to enhance collective impact within communities. 
  • Increase policy and environmental changes to support healthy eating and active living behaviors. 

Methods- MAPPS for Change

The HLI methods provides a framework for community collaboration and alignment across three sectors (Community, Healthcare, Government). These methods are based on approaches that contribute to lowering rates of obesity and include: 

  • M- Consistent use of message - 12345 Fit-tastic!
  • A- Consistent assessment of weight status and behaviors.
  • P- Customized healthy lifestyle plans for all. 
  • P- Policies and environment that enable healthy eating and active living. 
  • S- Statistics for collecting and analyzing key measures to track progress and support process improvement. 

What's been accomplished? 

  • Partners have designed and launched a 12345 Fit-Tastic! message campaign. (2014 Summary Report)
  • Established a protocol for healthy weight assessment/customized plans in primary care. 
  • Expanding efforts in the policy arena.
  • Securing financial support to expand the reach of this effort. 
  • Enlisting more partners for a broader reach and impact.
