CMH requisitions can be obtained from our Client Services Section at (816) 234-3835 or through the links below. Additionally, we will accept legible written orders placed on a prescription pad with hand written legible signature. CMH requests that all laboratory test orders sent to us on or after 1/1/2011 contain a hand written legible signature or electronic signature of the referring physician.

The Department of Pathology and Laboratory Medicine will perform tests only at the written or electronic request of an authorized person. Direct-access testing or patient-authorized testing is not permitted.
