Student Observers

  • "Agreement for Student Participation/Observation" form would be placed here for the student to print off. Fill out designated areas ahead of time and bring to the observation. The staff member responsible for the student's observation time would then complete the remaining areas of the form with the student.
  • There would be a link for students to be able to read and review required hospital policies as outlined on the form.
