How to Apply

Adobe Reader is required to view the following forms. If you are using an Apple (Mac) computer, you will be unable to submit them electronically. Please save a copy of the forms on your computer, fill in the information and e-mail them to volunteer@cmh.edu.

Step 1

Fill out the Online Application. Note: Only completed applications will be processed.

Step 2

• Health Form (must be signed by applicant)
• Please provide a copy of your updated immunization record with proof of Tdap (not Dtap) and fax all immunization records to (816) 460-1005. View required immunizations.

Required for individuals under 18 years old

• Parental Permission Slip. Fax signed form to (816) 460-1005.

Next Steps

Once we receive your completed forms, you will be invited to an interview and scheduled for New Volunteer Orientation – if placement is found. You will also receive information regarding a blood draw for a required tuberculosis screening.
