Year 2 Progress Updates

Children’s Mercy is proud to participate in efforts to bring solutions to some of our region’s most pressing issues that affect the well-being of children. In 2013, Children’s Mercy conducted a community health needs assessment and identified three priority needs. Below is a summary of the identified priority needs with a listing of the work Children’s Mercy is supporting to address the needs. 

Need #1: Access, Including the Importance of a Medical Home

 

  • The Beacon Program is seeing 100+ complex, chronic, special healthcare needs patients and their siblings.

  • Progress made in supporting the development of medical homes within CMH Clinics as well as community-based practices. 

  • Provide on-site health services at Operation Breakthrough, University Academy, and Synergy Services.

Need #2: Injury and Safety

 

  • Support Promise 1000, a community-wide program to coordinate home visit services across the region.

  • Developed the Safe and Healthy Families Clinic, a Trauma Prevention and Treatment Program for children who experienced a traumatic event. 

Need #3: Nutrition, Weight Status, and Physical Activity

 

  • The Weighing In Coalition brings together a diverse set of partners working to improve healthy eating and active living throughout the Metro area. 

  • The Zoom to Health program provides weight management services and activities to families and children at area YMCAs.

Note: The Year 3 Implementation Report (available June 2016) will provide a summary of the programs and activities that address each of the identified needs and the outcomes achieved since 2013.
